Once you or your loved one is home after a hospital stay of surgery, our team will work towards preventing re-admission to the hospital and complications that could lead to a health emergency. A registered nurse will be assigned to manage your loved one's health.

Upon discharge, Best Option Health Services Inc. Case Managers, work in partnership with insurance case managers, social workers, doctors, therapists, and hospital discharge planners to make certain that the utmost benefits are obtained for each client. Closely working with everyone in the care management of our clients is a top priority. Once our patients are returned from hospitalization to the comfort of their homes, we continue to work on ensuring that care and benefits are maintained. To maintain continuity of care with skilled nursing, our agency staff works closely with Durable Medical Equipment (DME) companies, to organize any necessary medical equipment and supplies needed.
